Case Management Services are available for individuals 16 years of age and older
who have more persistent or severe mental health symptoms, and who require more support
with their day-to-day activities, or require support in a number of areas of their
lives such as housing, finances or physical health.
Family members who support an individual facing a severe mental health issue can
also have access to counselling services.
Our Case Managers offer you support and encouragement and assist you in achieving
your goals to acquire or maintain independence and satisfaction in your every day
After an initial assessment and the identification of your needs and goals, our Case
Managers with you, will determine which supports are needed to achieve your goals.
They will assist you in developing a recovery or wellness action plan that is centered
on your needs and specific circumstances.
Case Managers can offer assistance in the following areas:
Education on your condition (or diagnosis), medication and symptoms management;
Monitoring symptoms and side effects;
Skills teaching such as budgeting, grocery shopping, and other life skills;
Referrals and linkages with other community services;
Advocacy to ensure the protection of your rights to access income and accommodation;
Crisis and relapse prevention.
